Waitlist Opening

This waitlist is an administrative intake tool for individuals seeking future attachment to a primary care provider at South March Medical. It is used to organize requests for care and to contact individuals as physician capacity becomes available, including when new physicians join the clinic.

Submission of this form does not establish a physician–patient relationship, does not constitute acceptance to the clinic, and does not guarantee that care will be offered. A physician–patient relationship is formed only after direct contact from the clinic and formal acceptance by a provider. Until that time, you remain responsible for obtaining medical care through your current provider or other available health services.

This waitlist is not monitored for clinical concerns and must not be used for urgent or time-sensitive medical matters. If you have an immediate health issue, contact your current healthcare provider or seek urgent or emergency care as appropriate.

All sections of the intake form must be completed in full. Incomplete submissions cannot be processed or retained. Accurate and complete information is required to allow the clinic to contact you if capacity becomes available.

 

We recognize the urgent need for primary care in our area and are working hard to accommodate new patients.
